Jimmy is still alive!? After the whore house episode I thought he was died. Although I guess it would explain why Cas was so nervous, Cas didn't want to have sex because Jimmy was still inside, but he couldn't tell Dean no because the last he did that Dean said they were done.

I thought so too, although I'm not sure how much it actually contributed to Castiel's hesitance. If he really thought it was doing wrong by Jimmy, he wouldn't have given in so easily, even to Dean. This wasn't some kind of important moral issue like the last one was, Dean would never have cut him off for that.
